Daughter's Thal Bloodwork

Well I finally got the blood work results for my daughter (these are the results that the doctor based her thalassemia trait diagnosis on). She's 2.

I'll only list the flagged results--let me know if any of you want a more complete list.

RBC 5.20 (high)
MCV 63 (low)
MCH 20.5 (low)
RDW 21.9 (high)
Platelets 476 (high)
Neutrophils 20 (low) 1% bands

Everything else checked out fine and the hematologist commented on her blood cells in the following manner:

Moderate microcystosis.
Moderate anisocytosis and poikilocytosis
Slight hypochromasia
Slight polychromatophilia

Which of course means many of her rbc are small, pear shaped, while some are large. WIth some paleness and bluish coloured cells.

Now tell me because the doc didn't (our ped)--should I do anything for her extra. They didn't include a ferritin test (sigh)--so I am not sure if she is actually iron deficient. Should I follow up with a hemotologist for a more complete work up? And as for the platelet count being high how much of that should be a concern (my sons-non thal diagnosis) are in that range as well.

On MY front: I ams till waiting for my blood work results to come through. I had my free t3 and t4 test along with my adrenals and my complete CBC and ferriten test. I am still exhausted and cranky and what not...and I am hoping for something to come back because I hate to think it's all in my head.

Hallo Birdyknits,

the results are typical for a moderate thalassemia caused by an genetic mutation. It dosent show it is an thal minor or intermedia. Therfor you would an DNA analysis.
Does your daughter has any symptoms?
Maybe she has an exessiv erythropoiesis, this could explane the high RBC. It would be helpfull to know her ferritin and total iron bounding. So you can see if sche is deficient or absorbe to much of iron.

Let's see your tests because it looks like your child could be too young to show signs of a mild to moderate anemia that may become more evident as she ages without some vigilance. That is a good thing though. I agree with Bernstein that you need the iron studies since she does show signs that could be iron deficiency or mild thalassemia. Also, the hemoglobin electrophoresis is only useful usually for very classic cases of major anemia. Looks like they are also testing her for polycythemia and making sure the sickle test isn't a false negative. If you can afford the DNA test, do it since she will one day want her own babies and should know what her and her partner are up against. Hopefully by then, it will be a matter of some miracle injection that protects your fetus from hemoglobinopathies...who knows what modern medicine will figure out. They can't cure a cold but they can transplant hearts so lets not lose all hope...:)

It is hard to diagnose some things in babies and young children. Her platelets are a little high but not so much that it isn't just a lab, age or regional deviation. The low MCV signals iron deficiency or thal trait. You will need that iron test to know how to raise her from a dietary perspective and your tests for yourself and it looks like as long as you take care of her diet and teach her what she needs and how to stay hydrated, she will be fine and she will be grateful her mom was on top of everything and not discover in her thirties why she has been dragging her feet all her life like my dad did and many here :)
